Note: This form is not to request a visa appointment with the Italian Consulate. Rather, this form is to confirm that you have made an appointment with the consulate, as students cannot attend the Cornell in Rome program without a visa or active European Union passport.
You need to request an appointment with the consulate directly. Please remember that you must go to either the Italian consulate in New York City or the Italian consulate servicing your home address.

You must submit your visa application in person and by appointment only with the Italian Consulate. They do not have drop-in hours. The Italian Consulate requires you to make an appointment online for a date no earlier than 90 days before your fall arrival date. Appointments fill up quickly, so please do not wait — students who have waited until May to make an appointment often find that none are available.
Remember that you will need to relinquish your passport for the entire processing time with the consulate, so plan accordingly if you will be traveling. Typical processing time is one to two weeks, although it varies by consulate.
